Abstract

Dengue fever in children remains an important health problem due to the risk of rapid progression to a critical phase. The neutrophil-lymphocyte ratio (NLR) is an easily obtainable inflammatory marker that has been proposed to reflect disease severity, although previous findings remain inconsistent. This study aimed to evaluate the correlation between NLR and the severity of dengue fever in pediatric patients. A cross-sectional study was conducted using 48 medical records of children aged 1–18 years who were diagnosed with dengue fever and treated at UKI General Hospital from January 2023 to October 2025. The sample was picked by using consecutive non-random sampling. The analyzed variables included neutrophil count, lymphocyte count, NLR value, and disease severity classified according to the World Health Organization 2009 criteria. Statistical analysis was performed using a Spearman correlation test, with p < 0.05 considered statistically significant. The majority of patients were male, with a mean age of 9.29 years. The mean NLR value was 3.28, with a median of 2.59 and a range of 0.28–10.38. Most cases were classified as dengue with warning signs, and no cases of severe dengue were identified. Correlation analysis showed no significant association between NLR and dengue severity (r = 0.094; p = 0.527). In conclusion, NLR was not significantly associated with disease severity and therefore cannot be used as a single predictive indicator of dengue fever severity in pediatric patients.
